Urine Samples Being Left for Testing

Reception will only accept urine samples that have been requested by a clinician.  If you feel you have a urine infection please book an appointment at reception.

Please be aware that any samples left in the post box will not be tested.

NON NHS WORK

There will be a fee for all non NHS work including forms/medicals.  Please call the surgery and ask to speak to the secretary who will be able to give you a price and an idea of timing or will advise you to book you an appointment if necessary.

Travel Questionnaire Form

If you require any vaccinations relating to foreign travel you must fill in a "travel questionnaire" form well in advance of your departure date. This will include which countries and/or areas within countries you are visiting to determine what vaccinations are required and which vaccines you have already had. 

We will contact you to make an appointment when the nurse has the information needed. It is important to make this initial appointment as early as possible - at least 6 weeks before you travel. The travel vaccines have to be ordered as they are not a stock vaccine.

*Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

• Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
• Insurance claim forms
• Passport signing
• Prescriptions for taking medication abroad
• Private sick notes
• Vaccination certificates
• Certain letters

The fees charged are based on British Medical Association (BMA) suggested charges.

Antenatal Care
The care of patients attending for antenatal services will be shared by the community midwife, health visitor and one of the Doctors.
