Church Hill doctors' surgery in Pulham Market provides the highest quality healthcare to patients in the area. Our doctors, nurses and all our other staff are dedicated to offering a professional service and this surgery website will help us to keep all our patients up to date with news and information about our practice.

Where to find us-

Church Hill Surgery – Station Road, Pulham Market. Station Road is in the centre of the village of Pulham Market opposite the village green. The surgery is situated 50 yards down this road on the right hand side. These premises have suitable access for patients with disabilities.

NON NHS WORK

 There will be a fee for all non NHS work including forms/medicals.  Please call the surgery and ask to speak to the secretary who will be able to give you a price and an idea of timing or book you an appointment if necessary.  


Information for Patients Regarding Named GP

   All patients will be assigned to Dr Mondol

Please be aware that this does NOT restrict you from seeing your preferred GP.

You can still book an appointment with the doctor you prefer to see.

       


The NHS Friends and Family Test

The NHS Friends and Family Test is a way of gathering your feedback, so we can continually review our service.

It is based on one simple question: "How likely are you to recommend our GP practice to friends and family if they needed similar care or treatment?"

Your feedback will help us learn more about what you think of your experience - what you like and what you think we could improve. Ultimately, you're helping us to make changes that will ensure we can offer the possible care.

The NHS Friends and Family Test feedback cards will be available in the surgery from Monday 1st December 2014.

Whilst you are in the surgery, we would be grateful if you could take the time to fill one of these in and post it in the box in Reception.

You can also fill in a feedback card on the website. It can be found on the right hand side of the screen under "Have your say"

IMPROVED ACCESS

Our Practice is engaging in a pilot scheme providing improved access for patients to a range of healthcare professionals.  Working within a group of eight local Practices, we will be offering a range of appointments each weekday evening between 6.30pm and 8pm and during selected times at weekends, to registered patients, in at least one of the participating Practices.  Please contact our reception team for more details.

SEASONAL FLU CLINICS FOR 2018

 

We are running flu clinics differently this year as the Government have changed the flu vaccination that Practices provide to patients aged 65 and over (those aged 65 by the 31/03/19)

The vaccine for 65 years and over is called Fluad and is likely to give better protection against flu for those aged 65 and over.

 

This year delivery of fluad delivery is being staggered so we will not have sufficient supplies to vaccinate everyone during September or October so we have been advised to prioritise who we provide the vaccinations to as follows:

Priority 1-Those aged 75 years and over

Priority 2-Those aged 65 -74 years old

Flu Clinic for over 75year olds

We have the first Flu clinic running for 75 years and over on Monday 24th September 9am-5pm, and Thursday 27th September 2-5pm.  Please ask reception for more dates.  You will need to book an appointment.

Flu Clinic for those aged 65 years and 74 years old

We will be running a flu clinic on Monday 5th November.  Please speak to reception to make an appointment, additional dates will follow.

Flu Clinic for those aged under 65 years who are in a clinical risk group

We will be running flu clinics on Monday 29th October.  Please speak to reception to book an appointment.

Housebound patients

Please let reception know so we can arrange these to be done at your home.

 

We apologise for any inconvenience caused.  The delivery of flu vaccines is completely outside of our control.
